Gillingham & Associates has issued its agency profile survey form for 2008. This form lets us have the latest and most accurate information available about your agency, including contact information, E&O Carrier information, and copies of Agent/Agent Residency Licenses. If you are an agent doing business with us, please download this form (Word .doc), fill it out, and mail, e-mail, or fax it to our office before the end of 2007.
